Abstract

AIM: There is a growing movement in favor of Fine Needle Aspiration Biopsy(FNAB) over excisional biopsy among clinicians at our hospital in diagnosis of solid breast lesions. We evaluated the trend of procedure selection in the pathologic diagnosis of solid breast masses at our institution. MATERIAL AND METHOD: 4856 breast specimens (all surgical specimens, FNAB, nipple discharge, cyst aspirations, etc.) from 4556 patients in our archive between 1996-1998 were rewiewed retrospectively. RESULTS: There were 4856 breast specimens at our arehive between 1996-1998. 3807 of them were cytological specimens out of which 2909 (76%) were FNAB. 1049 (23%) were surgical biopsy material, out of which 674 were excisional biopsies. Diagnosis in the excisional material were as follovvs: Fibroadenoma 226, gynecomastia 43, local recurrence 28, and intraductal papilloma, mastitis, etc. 35. These excisional specimens were all from curative surgical procedures. From 448 excisional biopsies; 242 (54%) had FNAB before surgery. The results of FNAB were; fibroadenoma in 193, gynecomastia in 5, local recurrence in 9, intraduetal papilloma, mastitis in 35 cases. The true number of the diagnostic surgical biopsy was 202. CONCLUSION: Our study reveals the fact that we only had 202 diagnostic surgical specimens compared to 2909 FNAB specimens in the last three years (FNAB/diagnostic excisional biopsy: 2909/202=6.9). The result indicates that FNAB is being chosen as the first procedure in the diagnosis of solid breast lumps at our institution.
